Masonic declaration book. This should be hopefully suitable to all needs. Custom versions including logo and lodge name can be created on request from the author (for an additional cost of course). As can versions for Royal Arch or other side degrees.
Declaration book to record details of new initiates, including declaration itself, signature, profession and address of initiate and the date of their signature, as well as space for witness to sign. Dates are also recorded in this book for date of Initiation, Passing and Rasing, as well as the date the certificate is received.
Book is hard back with dust cover, and measures 6" x 9" and contains space for 25 initiates.
Other books are available elsewhere, but ours represents excellent value for money and is a quality book used by many Lodges.
